WE DO REQUIRE A TRAVEL QUESTIONNAIRE TO BE COMPLETED AND RETURNED TO US BEFORE A TRAVEL APPOINTMENT CAN BE BOOKED.

If you require any vaccinations for foreign travel, you will need an appointment with a practice nurse to discuss your travel arrangements in order to determine which vaccinations are required.

Before booking the appointment, please complete and submit a 2018 travel risk assessment form this helps us offer you the best advice during the appointment. We will not be able to book a travel vaccination appointment until this has been submitted.

It is also important to make your initial appointment as early as possible – ideally at least 6 weeks before you travel – as a schedule of travel vaccinations may require more than one visit to the surgery.

If you have received any vaccinations at another surgery or travel clinic  please bring your own travel vaccination record to the appointment.
