Travel Clinics and Informaition

Patients planning trips abroad who may require any advice and/or vaccinations MUST complete and hand in to Reception a completed travel risk assessment form

On returning the form, you will be booked into a 20 minute appointment with one of our Practice Nurses at their earliest availability. Please return the form as early in advance as possible to ensure that you can be seen in time. We recommend a minimum of 6 weeks before your departure. If you are taking a trip in less than 6 weeks, we recommend that you visit a private travel clinic.


If you have received any vaccinations at another surgery or travel clinic  please bring your own travel vaccination record to the appointment.

Please note that some travel vaccines are not included in the services provided by the NHS and will result in a fee being charged. For information on this, please see Travel Invoice 2019
