How to Register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.


It takes a week to process your registration.

To register, you will need to bring ALL OF THE FOLLOWING FORMS COMPLETED; NHS Registration Form, New Patient Questionnaire Nov 2019, a  Patient Online Registration Form 16+ or a PatientOnline Registration Form for 12-15 age group and Summary Care Record 2018

Blank forms are also available at reception.

We will also need to see two forms of identification: either your passport, drivers licence or birth certificate plus a utility bill or other form of bill, that is less than three months old, that shows your current address. Please note that your forms can not be accepted without these.

We recommend that new patients who are on regular medication make an appointment to see a GP to ensure that all your repeat prescribing is set up ready for you to order medication.

Online Access

For online access allowing you online booking of appointments, repeat prescriptions, test results once they have been reviewed by your GP and a brief overview of your patient record please read the Patient Online Information Leaflet and complete the Patient Online Registration Form.

Please note the form must be delivered to surgery, it takes 1 week to set up your account and will need to be collected with photographic identification before the password can be issued to you.

After you have set up your online access and password you can download the Patient Access App to your device or access it online.
