How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

To register, you will need to bring a completed NHS Registration FormNew Patient Questionnaire, a PatientOnline Registration Form or a PatientOnline Registration Form for 12-15 age group and Summary Care Record 2018

Blank forms are also available at reception.

We will also need to see two forms of identification: either your passport, drivers licence or birth certificate plus a utility bill or other from of bill, that is less than three months old, that shows your current address.

Usually the best time to register is between after 12.00 noon ti 6pm Tuesday to Friday – if possible we would ask you to come between these times as reception is especially busy early in the morning and late in the afternoon which can delay us in helping you register. PLEASE NOTE WE DO NOT ACCEPT REGISTRATIONS ON A MONDAY.

We recommend that new patients who are on regular medication make an appointment to see a GP to ensure that all your repeat prescribing is set up ready for you to order medication.
