We have an open list and welcome requests for registration from patients living in or moving to the practice area.
To register, you will need to bring a completed NHS Registration Form, a New Patients Registration Form, (or a Child New patient Registration Form for children under the age of twelve,) a PatientOnline Registration Form or a PatientOnline Registration Form for 12-15 age group and a Summary Care Record
Blank forms are also available at reception.
We will also need to see two forms of identification: either your passport, drivers licence or birth certificate plus a utility bill or other from of bill, that is less than three months old, that shows your current address.
Usually the best time to register is between the hours of 12.00 noon and 3.00pm Tuesday to Friday – if possible we would ask you to come between these times as reception is especially busy early in the morning and late in the afternoon which can delay us in helping you register. Please not that we do not accept patient registrations on a Monday.
We recommend that new patients who are on regular medication make an appointment to see a GP to ensure that all your repeat prescribing is set up ready for you to order medication.
Health checks are also available with the practice nurse.
For online access allowing you online booking of appointments and repeat prescriptions please also complete the Patient Online Registration Form. You also have the option to use this service to view test results once they have been reviewed by your GP and have a overview of your patient record.
